Job Title: RELEASE OF INFORMATION CLERK

Job Summary:

Performs under the supervision of the HIM Supervisor. The Release of Information Correspondence Clerk responds to all requests for medical record information from various requestors including health care providers, patients, attorneys, third party payors, Disability, and subpoenas. All Release of Information must be in accordance with HIPAA federal regulations and state regulations. The Release of Information Clerk interacts with patients in person or by telephone and obtains appropriate authorization for release. To include any and all other duties assigned by the HIM Director and or Supervisor.

Essential Job Duties: 

  • Release medical information to patients, insurance companies, attorneys, physicians , other healthcare facilities, various hospital departments and pursuant to Subpoenas or Court Orders

  • Collects, distributes and open all mail on a daily basis. Stamps date of receipt on all mail

  • Enters all correspondence request into the computer system, completes all required fields

  • Review authorization and request of records for validity; verify records in the electronic medical  record to ensure information released  matches the name of the patient on the request or authorization

  • Avanti Hospitals’ Health Information Management Department will provide all patients  who request an electronic copy of their health information within 3 business days

  • Coordinates with copy service to ensure all records requested are available in paper or a CD is prepared

  • Logs RAC records on the MetaData sheets for submission to CMS with requested information copied and placed on a CD to be sent in a timely fashion

  • Performs other duties assigned to me by my Director/Supervisor

  • High school graduate or some college course work

  • Working knowledge of HIPAA rules and regulation and consent laws

Licensure/Certifications:  N/A

ADA/Physical Demands:
