Job Title: LVN Case Manager – Case Management/Social Services


Job Summary:


A Licensed Vocational Nurse that is responsible for assisting with the management and coordination of care for patients in the acute setting. Job duties include utilization review, discharge planning, assisting the RN Case Manager with care coordination, assisting with efficient patient throughput, some variance management such as length of stay, and ensuring payment for services is obtained. The LVN Case Manager follows regulatory standards and insurance company guidelines for the purpose of reimbursement. The LVN Case Manager reports to the Manger or Director of the department or the R.N. Case Manager Lead if established.


Essential Job Duties: 



  • Demonstrates ability to collaborate with patients / significant others as well as other members of the health care team in determining the appropriate level of care, utilization of resources and length of stay (utilizing Interqual criteria)

  • Demonstrates knowledge of, and ability to, perform concurrent review of patient treatment plans in accordance with the hospital policies and third party reimbursement systems

  • Works with the RN Case Manager, Discharge Coordinator and Social Services regarding discharge needs and final throughput.

  • Applies appropriate clinical knowledge, per scope of practice, in order to identify patient needs and effectively communicate medical necessity and treatment plan to payers.

  • Anticipates and acts upon barriers to efficient and effective patient progression by identifying clinical, operational, financial and social issues affecting patient outcomes. Intervenes, or seeks guidance from the RN CM or the Department Director as needed.

  • Serves as a role model for the Mission, Vision and Values of the organization and fulfills other job duties as requested within Scope of Practice

  • Maintains current competencies as well as updated knowledge in specific area of practice.

  • Prevents denials and disputes by communicating with payers and documenting relevant information.

  • Identifies patients at risk for extended lengths of stay, readmission, and complex discharge needs. Works collaboratively with the RN CM, the Discharge Coordinator and Social Services to overcome potential discharge delay barriers.

Job Requirements


Education/Experience:



  • Minimum of 2 years LVN Case Management experience in the Acute Care setting.


Licensure/Certifications:



  • Current LVN licensed in good standing by the California Board of Nursing

  • Current BLS for Health Care Provider card
