Job Title: ED ABSTRACTOR

Job Summary:

Performs under the supervision of the Director of Health Information Management.  Responsible for providing timely and accurate abstraction of Emergency Department (ED) services records to facilitate accurate reimbursement, and EM Levels.  This position is responsible for, but not limited to, charge capture for the Emergency Department and data mining. To include any and all other duties assigned by the HIM Director and or Supervisor.

Essential Job Duties: 

  • Captures documentation found on the ED Flow sheets, and verifies medications, tests and procedures located on the ED T-Sheets documentation by the Nursing Staff and verifies this information from the ED Physicians T-sheets while computer assisted coding tool integrates points for services rendered. Abstracts appropriately information found within the ED record into the Computerized EM Coding System.

  • In-depth knowledge and understanding of nursing documentation

  • Utilizes available resources appropriately to maintain quality and consistency in abstraction, EM level capture.

  • Follows a defined process to query the Nursing Staff for completion and/or clarification of documentation necessary to ensure coding compliance and accuracy.
  • Notifies HIM management if inappropriate or unethical documentation practices are identified or if there is uncertainty with legibility of the record, questionable, or unclear documentation.

  • Correctly identifies ED records that meet performance improvement initiatives. Provides feedback that identifies opportunities for documentation improvement and education.

  • Maintains departmental standards for productivity, accuracy, and quality

  • Performs other duties assigned to me by my Director/Supervisor

Job Requirements

Education/Experience:

  • High school graduate or some college coursework

  • Anatomy and Physiology

  • Medical Terminology

  • Coding Experience, but not required

  • Prefer at least 2 years of current experience in an acute care setting

Licensure/Certifications:  N/A
