Job Title: Behavioral Health Outpatient Coordinator

Job Summary:

Under supervision of the Director of Behavioral Health Services, handles all patient registrations.  Communicates professionally with Patient’s and or Family members to obtain all demographic and insurance information. Coordinates efforts with the Insurance plans for notification of medical services and verification of coverage. Has broad knowledge of HIPAA regulation governing all Patients PHI. Reviews all information obtained and validates accuracy. Coordinates the logistics for patients’ participation in the Outpatient Program, including transportation, meals, appointments with physicians, and other duties as assigned.

Essential Functions:

  • Participate in treatment team meetings and coordination of care as needed.
  • Coordinate meals and snacks for patients
  • Complete medical records including filing and form tracking
  • Entering patient intake and assessment information, collaborating with the clinician and nurse when necessary.
  • Schedule and initiate intake process including providing initial documentation and orientation to the program
  • Creating and maintaining a warm, personal rapport with patients.
  • Assisting the patient in completing intake packets or any other necessary forms.
  • Develop and/or maintain an organized process of scheduling healthcare providers in an extremely busy office.
  • Work within an electronic medical record system, both inputting information and obtaining information already in the system.
  • Practice compliance with all policies, record keeping requirements, checklist assignments, assigned documentation, and any other assigned duties.
  • Work in conjunction with each Outpatient program to ensure that PHP and IOP admissions are maximized and appropriate and that the admissions process is followed in a timely manner.
  • Track calls and document calls effectively and efficiently in call-log and Call Tracking Metrics
  • Work with program directors of PHP/IOP programs to schedule in-person assessments and coordinate admissions when appropriate
  • Ensure completion of the Census report.
  • Completing an intake call sheet and gathering all necessary demographic, clinical and financial information on call sheet. Ensures information is accurate and complete as call sheet is transmitted to hospital clinical and business office staff.
  • Ensures verification of insurance eligibility and benefits including primary and secondary payers.
  • Initiates welfare checks in addition to follow-up calls when clients fail to attend treatment or discharge from program to discuss client experience. 
  • Answer general questions and make referrals for families/professionals seeking a range of Behavioral Health services. 
  • Work closely with the Clinical team in assisting with referrals, data base tracking, transfers, and insurance prior authorizations for inpatient psychiatric care 
  • Establishing and maintaining positive working relationships by collaborating with internal and external referral sources
  • Maintains working relationship with administrative, operations, and clinical staff to assure efficiency in the day to day functioning of the resource and scheduling service. 
  • Utilize Electronic Medical Record and Scheduling software to assess the demand/waiting lists for services and the ability of current service capacities to meet the community’s needs
  • Coordinate patient transportation

Job Requirements

Experience/Education:

  • At least 2 years’ experience or combination education and experience preferred.
  • Bachelor’s Degree preferred

Skills/Characteristics:

  • Requires ability to multi-task and exhibit excellent analytical, interpersonal, problem solving, time management and communications skills

Licensure/Certifications:

  • CPR Certification
  • Management of Assaultive Behavior training
