Job Description and Job Responsibilites

Job Title:  Acute Care Nurse Practitioner

Job Summary:

The NP will utilize advanced clinical skills and knowledge to assess patients and manage all aspects of care for the capitated patient population. The NP will work collaboratively with attending physicians to ensure that continuity of care is maintained between inpatient care and referring physicians from admission to discharge. He/she will communicate clinical issues to the attending physician, write orders according to standardized procedures/protocols, coordinate care, and facilitate arrangements for discharge. The NP will interact with key ancillary departments such as pharmacy, radiology, and clinical laboratory. The NP will support/educate clinical nursing staff regarding treatment goals and strategies.

Essential Job Duties:

  • Contributes to physician's effectiveness by identifying short-term and long-range patient care issues that must be addressed; providing information and commentary pertinent to deliberations; recommending options and courses of action; implementing physician directives.
  • Assesses patient health by interviewing patients; performing physical examinations; obtaining, updating, and studying medical histories.
  • Determines abnormal conditions by administering or ordering diagnostic tests, such as x-rays, electrocardiograms, and laboratory studies; interpreting test results.
  • Documents patient care services by charting in patient and department records.
  • Order medications in accordance with hospital protocol / formulary.
  • Perform medication reconciliations.
  • Performs therapeutic procedures by administering injections and immunizations; suturing; managing wounds and infections.
  • Instructs and counsels patients by describing therapeutic regimens; giving normal growth and development information; discussing family planning; providing counseling on emotional problems of daily living; promoting wellness and health maintenance.
  • Provides continuity of care by developing and implementing patient management plans.
  • Initiate / coordinate discharge planning (medications, follow-up care, outpatient arrangements, durable medical equipment, home health services etc.) with supervising physician, nursing staff and discharge team.
  • Contributes to team effort by accomplishing related results as needed.




Job Requirements

Education/Experience:

Master’s degree in Nursing with at least 3 years of acute inpatient experience

Licensure/Certifications:

Required

Current California NP licensure.

Current Nurse Practitioner Furnishing License.

Verification of Experience to Furnish (must have experience furnishing for at least one year under MD supervision and must have a physician attest to the fact that such supervision took place).

Current CPR certification.

Current ACLS certification.

Preferred

One year of experience as an NP in an inpatient, emergency department, or outpatient adult medical setting.

 

Job Requirements
