Job Description and Job Responsibilites

Job Summary:

The Stroke/Sepsis Coordinator RN leads and facilitates the development, implementation and ongoing adherence to the identification and treatment of the stroke and septic patient population.

Essential Job Duties:

The Stroke/Sepsis Coordinator is responsible for all programmatic responsibilities including, but not limited to:

  • Ensures the evidence based screening and treatment protocols become the standard of care within the facility.
  • Collaborates with medical and hospital staff to provide multidisciplinary care to patients with a diagnosis of stroke, sepsis, severe sepsis and septic shock.
  • Reviews and analyzes clinical data and presents the data in an understandable and meaningful context to staff and physicians to motivate adherence to protocols.
  • Observes clinical processes and conducts surveillance activities, with immediate intervention (just-in-time) as indicated.
  • Forms idea for process improvement, develops and engages team solutions.
  • Provides education and clinical expertise to care team (physicians and staff), patients and families.
  • Collaborates with physicians and staff in an effort to maximize timely evidence based care to decrease morbidity, mortality, length of stay, and re-admissions.
  • Leads stroke/sepsis committee meetings with physician and nursing champions.
  • Leads hospital, medical staff and shared governance meetings focusing on stroke and sepsis as needed.
  • Participates in multidisciplinary rounds and proactively responds to patient needs to improve patient outcomes and positively impact overall patient experience.
  • Works in collaboration with the care team to develop and implement individualized teaching plans for patients and families.
  • Collects and analyzes stroke and sepsis program data to drive the programs.
  • Serves as a liaison between physicians and clinical documentation improvement (CDI) specialists relative to stroke or sepsis diagnoses.
  • Participates in hospital, division and corporate wide collaboration for stroke care and sepsis care and treatment.
  • Maintains knowledge and understanding of changing performance measurements for high quality patient outcomes.
  • Locates, accesses, and understands local and national best practice benchmarks related to patient experience and outcomes. Interacts with patients/families regarding experience.   For less than expected care experience, facilitates action and resolution with leadership, and provides timely written correspondence for case closure.

Job Requirements


  • Graduate of school of nursing required, MSN preferred. 


  • Licensed as a CA Registered Nurse required.
  • American Heart Association Basic Life Support (BLS) Health Care Provider required.


  • Minimum of 3 years of recent clinical experience.
