Job Description

Job Summary:

Codes hospital Observation, Day Surgery, and GI (Gastroenterology) medical records.  Assigns ICD-9-CM diagnosis and procedure codes along with CPT surgical codes, and applicable APCs or EAPGs for reimbursement and data collection needs. 

Job Duties:

  • Assigns ICD-9-CM and CPT codes for patient records for reimbursement and statistical data.  Identifies and assigns ICD-9CM primary and secondary diagnoses and procedures codes and CPT 4 procedural coders for Observation, Day Surgery, GI (gastroenterology) and other medical records as assigned, by using the encoder and Computer Assisted Coding application. Follows the Ambulatory Revenue Management  workflow for outpatient coding and accurately abstracts information from medical records.
  • Validates the CDM assigned CPT codes and appropriately assigns the modifier and addresses OCE, NCCI and charge capture edits. Assesses appropriateness of APC/ EAPG assignment.
  • Reviews and identifies any Local Coverage Determination Policy (LCD) issues with outpatient accounts and rectifies these issues.  

Job Qualifications:

  • Registered Health Information Administrator (RHIA), Registered Health Information Technician (RHIT), Certified Coding Specialist (CCS), Certified Professional Coder (CPC), or Certified Professional Coder-Hospital (CPC-H).  May be Registered Health Information Administrator (RHIA) eligible or Registered Health Information Technician (RHIT) eligible with credential obtained within the first six (6) months of employment.
